Step 1. Attend a Seminar

Step 2. Obtain Referral and Labs from Primary Care Physician
Please print out our physician referral form and list of required labs (blood tests). Take these to your primary care physician to be completed. The referral form and lab (blood test) results will be required at your initial evaluation with Dr. Umbach. By having your primary care physician request the labs, will save you money!!

Step 3. Patient Application
Log-on to our BPM (Bariatric Practice Manager) to complete a comprehensive patient history. From the information you provide, Dr. Umbach will be able to make a complete evaluation of your current health needs. The patient portal is also a helpful tool to you during your bariatric surgery journey. It will allow you access to your files and enable you to see where you are in the process, including viewing lab results.

Step 4. Insurance & Financial Options
Next, call the customer service number on the back of your insurance card to determine the bariatric benefit on your specific company policy. Whether you have insurance coverage for bariatric surgery or not, Blossom Bariatrics will help you every step of the way. Step 5.Schedule an Initial Evaluation
Finally, schedule an appointment with Dr. Umbach using our on-line scheduler. You and Dr. Umbach will determine which surgery is best for you – Lap Band, gastric bypass or Stomaphyx. Dr. Umbach can also help you if you need to undergo revision bariatric surgery in Las Vegas. Working together, you and Dr. Umbach will come up with a personalized game plan to get you to surgery. We encourage you to bring any support people, such as family and friends, to this appointment.
