Step 1. Attend a Seminar
Take the first step to a healthier you. It’s a FREE one-hour informational seminar where you will be able to meet and ask Dr. Umbach of Blossom Bariatrics questions.

For those who have attended seminars previously, please feel free to go directly to Step 2!!

Step 2. Insurance Verification & Schedule a Consultation
Next, call our office, at 702-463-3300, to provide our staff with your insurance information. We will call your insurance company for you to verify that your benefits include bariatric services. At this time we will also find out what the criteria is to obtain bariatric surgery through your insurance company. Once it is verified your insurance company covers bariatrics, a consultation will be scheduled with Dr. Umbach.

Those Lap Band Las Vegas patients with deductibles, out-of-pocket costs or without insurance, please inquire about finance programs available through our office.

Step 3. Patient Application
Create an account in the BPM (Bariatric Practice Manager) to complete a comprehensive patient history. From the information you provide, Dr. Umbach will be able to make a complete evaluation of your current health needs and put together a personalized list to get you to surgery. The patient portal is also a helpful tool to you during your bariatric surgery journey. It will allow you access to your files, enable you to see where you are in the process, including viewing lab results and schedule appointments.
